SoftWave vs Focused Shockwave vs Radial Pressure Wave Therapy

These terms get used interchangeably, but the devices differ. Here's what SoftWave refers to, how focused and radial treatments compare, and what to ask a clinic.
Short answer: they are related but not identical. SoftWave TRT is an unfocused electrohydraulic shockwave device. Focused devices concentrate energy at a point, and radial pressure wave devices (often called "radial shockwave" or EPAT) produce a pressure wave that is strongest at the skin and spreads out. No one machine is best for every diagnosis.
What SoftWave Refers To
SoftWave uses an electrohydraulic spark in water with a parabolic-style reflector to produce a broad, unfocused true shockwave field. That's the device we use in Solana Beach. Our SoftWave vs. other shockwave therapy page covers the technical differences in detail.
Focused and Unfocused Shockwaves
Focused devices (electrohydraulic, electromagnetic, or piezoelectric) concentrate energy at a target depth. Unfocused devices treat a wider area. Both produce true shockwaves; the treatment area and energy distribution differ.
Radial Pressure Waves
Radial devices use a projectile striking an applicator. Technically these are pressure waves, not true shockwaves, and their energy is highest at the surface. A review in the National Library of Medicine compares the two types.
Does One Always Win?
No. Evidence is tied to specific conditions, energy levels, and protocols, and head-to-head comparisons are limited. Some people improve with one type and not another; some don't improve with either. The Cleveland Clinic gives a general overview of shockwave therapy uses.
Questions to Ask When Comparing Clinics
- Which exact device do you use — focused, unfocused, or radial?
- Why do you recommend it for my diagnosis?
- What research supports that device for my condition?
- What's the full plan and cost?
Clinical practice guidance from the AAOS also mentions shockwave therapy as an option for persistent plantar fasciitis.
