Running and Exercise After Shockwave Therapy: Planning Your Return

A gradual, symptom-guided return usually works better than stopping completely or jumping back in. Here's how to plan it.
Short answer: most people can stay active during shockwave treatment, but a return to running or heavy training should be gradual and guided by symptoms. Avoid sudden jumps in mileage or intensity, and agree on a plan with your clinician.
A Simple Symptom Guide
Many rehab programs use a pain-monitoring approach: mild discomfort during activity that settles by the next morning is often acceptable; pain that's worse the next day means scale back. Research on pain-monitored loading is available on PubMed.
Building Back Up
- Start with walking or easy cycling
- Add short run-walk intervals on flat ground
- Increase volume gradually rather than all at once
- Keep strength work going
The American College of Sports Medicine and AAOS have guides on safe training progressions.
When to Hold Off
Hold off and contact your clinic if pain is increasing, you're limping, or you have swelling.
